Here at Physical Therapy at Bare Hills we strive to provide quality service to our patients! In order to do so, we first need to know a bit about you. For your convenience, we have attached a few quick and easy forms to fill out below (“New Patient Intake Packet” & “Verification Sheet”). These forms provide us with the ability to properly document your case, verify your insurance, and gather any information to better assess your injury. Feel free to print out the forms and bring them with you (completed), or you can fill them out on your computer and E-mail them to email@example.com.
-These forms provide us with basic background information including your name, date of birth, health history, etc.
-This form allows us to verify your insurance benefits prior to your initial visit.

WHAT TO BRING WITH YOU!
We require photo I.D. (preferably Driver’s License) and your most up to date insurance card. Upon your arrival, a copy of both cards will be made for our records. You will also need to bring any referrals that may have been provided to you by a primary care physician.
WHAT TO WEAR!
In most cases, we have our patients moving around quite a bit. With that being said, we strongly suggest wearing something comfortable. For example, we would recommend wearing exercise attire i.e. ( t-shirt, shorts,sweatpants). If for whatever reason you aren’t able to bring the recommended attire, no need to fret. We won’t have any problems working around it!
WHAT TO EXPECT ON YOUR INITIAL VISIT!
Once you have completed the paperwork, your physical therapist will review your medical history in order to effectively evaluate your injury. The initial evaluation process generally takes about an hour and will include several diagnostic tests/exercises in order to assess strength, range of motion, pain level, balance,endurance, and any functional deficits that may be present.
